Parent/Guardian Consent

I hereby give my child permission to participate in programing, activities, and initiatives included with Regality Inc. I give permission for my child to be photographed for marketing and record purposes. I am the natural parent or legal guardian having legal custody of said child. In consideration of my child participating in the Regality Treasure Within program, I hereby voluntarily release and agree to hold harmless DaVonne Kramer and Regality volunteers against any and all liability, claims, demands, actions, damages, expenses and costs that may result in my child’s participation in the Treasure Within program.

In the event that I cannot be reached in an emergency, I hereby give DaVonne Kramer and/or Regality volunteers permission to arrange, secure, and/or administer treatment including hospitalization for the participant listed above. I hereby give DaVonne Kramer and/or Regality volunteers permission to order x-rays, routine tests, treatment, to release any records necessary for insurance purposes, and to provide or arrange necessary related transportation for my child.
